Purpose of processing personal data<\/h2>\n<p>Personal data are processed in SYKETT\u00c4 Physical Activity Counselling for the following purposes:<\/p>\n<ul>\n<li>Management of customer relationship<\/li>\n<li>Physical Activity Counselling<\/li>\n<li><span style=\"background-color: transparent;\">Keeping statistics<\/span><\/li>\n<\/ul>\n<p><span style=\"color: inherit; font-size: 36px;\">6. Legal basis of processing personal data<\/span><\/p>\n<p><span lang=\"EN-GB\">Processing of personal data is based on Article 6 or 9 of the General Data Protection Regulation:<\/span><\/p>\n<p>\u2612<span lang=\"EN-US\">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;<\/span><span lang=\"EN-GB\">consent of the data subject<br>\u2612<span lang=\"EN-US\">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; implementation of the controller\u2019s or third party\u2019s legitimate rights. The l<\/span><span lang=\"EN-US\">egitimate right in question is customer relationship.<\/span><\/span><\/p>\n<p>These can be found from EU\u2019s General Data Protection Regulation, Article 6, paragraph 1, points a-f.<\/p>\n<h2>7. Categories&nbsp;<span lang=\"EN-GB\">of personal data and the data retention period<\/span><\/h2>\n<p>When registering to SYKETT\u00c4 Physical Activity Counselling, the following personal data is asked from the user via cooperation agreement on paper:<\/p>\n<ul>\n<li>First Name and contact information<\/li>\n<li>&nbsp;Last Name<\/li>\n<li>Signature<\/li>\n<li>Location and Date<\/li>\n<\/ul>\n<p>During the Physical Activity Counselling, the following personal data is filled together with the Physical Activity Counsellor to electronic preliminary information form:<\/p>\n<ul>\n<li>First Name<\/li>\n<li>Last Name<\/li>\n<li>Year of birth<\/li>\n<li>Phone number<\/li>\n<li>University and\/or working status<\/li>\n<\/ul>\n<p>In addition during the Physical Activity Counselling, the following data is filled together with the Physical Activity Counsellor to electronic form:<\/p>\n<ul>\n<li>Health status related to physical activity<\/li>\n<li>Physical activity habits<\/li>\n<li>Lifestyle habits<\/li>\n<\/ul>\n<p>The final report for the Physical Activity Counseling is completed together with the Counselor. The counseling process concludes with the completion of this report. Following the counseling, the client completes a feedback form, a first follow-up survey after three months, and a second follow-up survey after six months.<\/p>\n<p>The forms and the final report are retained for a maximum of one year after the counseling period ends.<\/p>\n<p>For statistical purposes, the following information is retained in anonymized form:<\/p>\n<ul>\n<li>health information related to physical activity, physical activity habits, and lifestyle choices from the initial questionnaire<\/li>\n<li>responses from the final report<\/li>\n<li>responses from the feedback survey<\/li>\n<li>responses from the follow-up surveys<\/li>\n<\/ul>\n<p><span style=\"color: inherit;\"><br><span style=\"font-size: 36px;\">8. Information systems that are used in processing<\/span><\/span><\/p>\n<p><span lang=\"EN-GB\">Personal data, that is collected in the SYKETT\u00c4 Physical Activity Counselling, are used in the following information systems:<\/span><\/p>\n<ul>\n<li>Google Drive\n<ul>\n<li>Preliminary information form, final report, feedback survey, and follow-up survey<\/li>\n<li>Physical activity counselor\u2019s records of sessions<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h2>9. Where is the personal data required in processing received from?<\/h2>\n<p><span lang=\"EN-GB\">Personal data is acquired from the data subject themselves. Providing personal and background information is a prerequisite for the participation of Physical Activity Counseling.<br><\/span><\/p>\n<h3 dir=\"ltr\" style=\"color: #1a1717; font-family: Jost, sans-serif; font-weight: bold; line-height: 1.8; font-size: 21px; margin-top: 0pt; margin-bottom: 0pt; padding: 5pt 0pt 11pt;\"><span style=\"color: inherit; font-size: 36px; font-weight: 400;\">10. Use of cookies<\/span><\/h3>\n<p>Cookies are used in the browser-based information systems of data processing. A cookie is a small text file, which is stored on the user\u2019s device by the browser. Cookies are used for carrying out services, making it easier to log into services and making it possible to compile statistics of the service usage. A user may restrain the use of cookies in the browser system, but this may prevent the system from operating properly.<\/p>\n<p>Cookies are used in the following browser-based data processing systems: Google Forms and Google Drive.<\/p>\n<p>Users can block the use of cookies in their web browsers, but this may prevent the system from functioning properly.<\/p>\n<h2>11. Regular transfers and disclosures<\/h2>\n<p><span lang=\"EN-GB\">Personal data are disclosed regularly for the following parties:<\/span><\/p>\n<p><strong>Student Union of the Karelia University of Applied Sciences, POKA<br><\/strong><span lang=\"EN-GB\">Student Union of the Karelia University of Applied Sciences, POKA, operates as a processor of the SYKETT\u00c4 user register\u2019s personal data. One SYKETT\u00c4 staff member is on the payroll of POKA.<\/span><\/p>\n<p>Karelia-ammattikorkeakoulun opiskelijakunta POKA<br>Tikkarinne 9<br>80200 Joensuu<br><a href=\"https:\/\/www.pokapoka.fi\/english\/become-a-member\/privacy-statement.php\">https:\/\/www.pokapoka.fi\/english\/become-a-member\/privacy-statement.php<\/a><\/p>\n<p><b>FSHS and University of Eastern Finland study psychologists<br><\/b>Clients are referred to physical activity counselors via the FSHS and the study psychologists at the University of Eastern Finland. Following the counseling session, information regarding the outcome and implementation of the counseling is provided to the FSHS. Data transfer or disclosure to outside EU or EEA and the legitimate grounds for such actions<\/span><\/p>\n<p align=\"left\"><span lang=\"EN-GB\">Personal data on the SYKETT\u00c4 website and mobile app shall not be transferred to outside EU or EEA. Personal data located in Google Drive are transferred to outside EU. Google obeys the standard contractual clauses of the European Commission in guaranteeing the required level of data protection, when personal data are transferred to outside EU. ISYY has agreed to EU\u2019s standard contractual clause in the legal clauses of its contract\u2019s data protection with Google.<\/span><\/p>\n<h2>13. Principles of protecting personal data<\/h2>\n<p><b><span lang=\"EN-GB\">Protecting personal data on Physical Activity Counselling<\/span><\/b><\/p>\n<p>Client cooperation agreement forms related to exercise counseling are stored in paper format in a locked area accessible only to exercise counselors and SYKETT\u00c4 staff during the counseling period and for one year after the counseling ends.<\/p>\n<p>Access to electronic exercise counseling forms is restricted to exercise counselors and SYKETT\u00c4 staff. Electronic forms are deleted one year after the exercise counseling ends.<\/p>\n<p><span style=\"color: inherit;\"><span style=\"font-size: 36px;\">14. Automated decision-making<\/span><\/span><\/p>\n<p><span lang=\"EN-GB\">Automated decisions shall not be made.<\/span><\/p>\n<h2>15.&nbsp;Rights of a data subject<\/h2>\n<p><span lang=\"EN-GB\">As a data subject you have the right to:<\/span><\/p>\n<ul>\n<li><span lang=\"EN-GB\">Inspect your data<\/span><\/li>\n<li><span lang=\"EN-GB\">Claim for rectification of your data<\/span><\/li>\n<li><span lang=\"EN-GB\">Claim for erasure of your data<\/span><\/li>\n<li><span lang=\"EN-GB\">Claim for restriction of processing<\/span><\/li>\n<li><span lang=\"EN-GB\">Object processing<\/span><\/li>\n<li><span lang=\"EN-GB\">Request for transferring your personal data from the controller to another controller<\/span><\/li>\n<li><span lang=\"EN-GB\"><span lang=\"EN-GB\">Withdraw your consent, if the processing of the personal data is based on the consent given by you\n<p><\/p><\/span><\/span><\/li>\n<\/ul>\n<p><span lang=\"EN-GB\">As a data subject you may use the above-mentioned rights by contacting the Data Protection Officer of the Student Union<\/span><span lang=\"EN-US\">.<br><\/span><span lang=\"EN-GB\">A data subject has the right to make a complaint to the Office of the Data Protection Ombudsman, in case the data subject deems that the valid data protection legislation has been violated in processing the personal data concerning the data subject.<\/span><\/p>\n<p>Advice and instructions in matters related to the rights of data subject are given by the Data Protection Officer.&nbsp;<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t","protected":false},"excerpt":{"rendered":"<p>NB: This document is a translation from the original document.
